Understanding our assessment process

Focussing on individual needs

Once you’ve had a look around our home and feel it’s the right place for your loved one, we’ll arrange a pre-admission assessment at a time that suits you.

This can take place at home or in hospital, whichever feels most comfortable. This helps us gently understand your loved one’s needs in advance, so their move into our home feels calm, reassuring and well supported from the very beginning.

Care that reflects needs, wishes and personality

During the assessment, we’ll talk through care needs, any equipment required, and the things that bring comfort and enjoyment, such as bathing preferences, favourite foods, hobbies and interests.

From this, we create a personalised care plan, which is regularly reviewed and adapted, ensuring care continues to reflect your loved one’s needs, wishes and personality.

Caring for your loved one, their way

Our assessment is supported by “This is Me”, a simple and trusted document created by the Alzheimer’s Society.

This helpful leaflet allows families to share important details about a person’s life, background, routines, and preferences—especially when communication may be difficult. By learning about meaningful moments, familiar comforts, and daily habits, we can truly understand who your loved one is. This supports person-centred care, helps reduce distress, and ensures care feels familiar, respectful and reassuring from the very beginning.

After the assessment, we’ll be in touch to let you know if our home can meet your loved one’s needs.

If it’s the right fit, you’ll have the opportunity to meet our teams, and our home managers will be there to answer any questions you may have. We’ll work closely with you to plan the move and help your loved one settle in, ensuring the transition is as smooth, reassuring, and comfortable as possible from day one.
